32K admission records (resident admit note) from the department of neurology, endocrinology, gastroenterology, respiratory and cardiovascular at our hospital between 2016 and 2018. Admission records are free text documents that are created by clinicians for each patient encounter, and they include general information, chief complaint, history of present illness, past medical history, physical examination, prehospital examination results and primary diagnosis.本数据集包含本院2016至2018年间，神经内科、内分泌科、消化内科、呼吸内科及心血管内科的32000份入院记录（admission records），此类记录具体为住院医师书写的入院病历（resident admit note）。入院记录为临床医师针对每一次患者就诊事件所创建的自由文本文档，涵盖一般资料、主诉（chief complaint）、现病史（history of present illness）、既往病史（past medical history）、体格检查（physical examination）、院前检查结果（prehospital examination results）及初步诊断（primary diagnosis）。
