This series consists of official colour-coded admission cards relating to the record of visitors and special information for reports (eg. Pensions Department, Child Welfare, Returned Soldiers, Overseas Cases). Details recorded are name of hospital; patient's name in full; date of admission; register number; date of birth; social condition; children; occupation; nationality; last address; religion; mode of admission (eg. T.P. by orders of S.M., order of S.M., certified, transfer); date of discharge/death; relevant information for reports, if any; names and addresses of visitors and their relationship to patient; name and date of each visit made.(19/15836-41). 6 boxes.
