Survey Data and Questionnaire for the Validation of the Urban Family Physician Program Model in Iran

This dataset contains the anonymized responses from a survey conducted to validate a conceptual model for the implementation of the Urban Family Physician Program (UFPP) in Iran. The survey was distributed among 387 participants with expertise in healthcare management, family medicine, and health policy. The dataset includes: An Excel file containing de-identified participant responses. A PDF file of the English version of the questionnaire used for data collection.
