Dataset for integrated community-based screening of communicable and non-communicable diseases in semi-urban Nigeria (2023-2025)

This dataset contains anonymized screening data collected during community-based outreach interventions conducted in semi-urban Nigerian communities between 2023 and 2025. The dataset includes variables on communicable and non-communicable diseases, including malaria rapid diagnostic test results, blood pressure measurements, hepatitis B surface antigen (HBsAg) status, random blood glucose levels, and prostate assessments. Data were collected as part of routine service delivery activities, and no identifiable personal information is included. Participation was voluntary, and informed consent was obtained prior to screening. The dataset is provided to support transparency, reproducibility, and further research in community-based disease screening and public health interventions in low-resource settings
