VPRS 17795 Disease Coding Cards

Each instance of a disease reported to the Fairfield Infectious Diseases Hospital (VA 1231) was recorded on the appropriate card, including the following information:- Disease code number- Diagnosis- Hospital number- Sex- Age- Result- Site- Etiology- Miscellaneous (any additional information recorded about the individual case)- Associated diseases- Admission date- Name (of patient)Some earlier versions of printed disease notification cards also contained the following (in addition to the above subheadings):- Manifestations- Operations- Physician (name of treating physician)费尔菲尔德传染病医院（Fairfield Infectious Diseases Hospital，VA 1231）接收上报的每一例疾病病例，均会被记录于对应专用卡片中，所涵盖的信息如下：疾病编码、诊断结果、住院号、性别、年龄、检测结果、发病部位、病因、其他补充项（针对该单一个案的所有额外记录信息）、伴随疾病、入院日期、患者姓名。部分早期印刷版疾病通报卡片除上述条目外，还额外包含以下内容：临床表现、手术操作、接诊医师（主治医生姓名）。
